Please note that if you will require vaccinations relating to foreign travel then you need to book an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. These should be given at least 2 months BEFORE you travel. In order to help us with this please download the Travel Risk Assessment Form below and hand it in to reception staff at least 4 weeks before your appointment with the nurse or fill the form in online (on this page) again at least 4 weeks before your appointment.

Please note that there may be a charge for certain vaccines. This is because not all travel vaccinations are included in the services provided by the NHS. The nursing team will be able to advise you of this once you have completed the form.
