How to register with us as a patient
Practice Catchment Area
You must live within the practice area which is shown within our practice catchment area page. You can enter your postcode (just below the map) to check if you live within our boundary.
If you are unsure, you can also contact reception to confirm.
If you do live in our practice catchment area then the quickest way to register is online, using the link below which will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
If you prefer, you can come to the practice and fill in/or collect paper registration forms. Alternatively you can download the National Paper Registration form here.
Registrations are usually processed within 2 working days but could take longer if we need to check anything with you from the information that you have provided.
Please be aware that if you already use the NHS App through your previous surgery, you will need a new linkage key from us so that the app can associate you with us. We will do our best to issue these when you register but again it can take time to process. This could affect your ability to access a Covid Pass on the NHS App so we do not recommend registering just before travelling in case you lose access to your Covid Pass.
You will be allocated a named GP on registration, but you will be registered with the practice rather than an individual GP. You can see any of the doctors here.
If you change your name and address or telephone number please inform us as soon as possible. If you move out of the practice area, you will be required to register with a new practice local to your new address.

In partnership with Doctors of the World UK, we will ensure our practice offers a welcoming space for everyone who seeks to use our services. We will ensure that a lack of identification, or proof of address or immigration status does not prevent a patient from registering at this practice. Don’t have documents? Don’t worry! We are a SAFE SURGERY for everyone in our catchment area. We might ask for ID or proof of address but if you don’t have any, and you live in our practice area, you can still register with us. We won’t ask for immigration documents.
- Everyone living in England has the right to free health care from a GP.
- If you find it difficult to communicate in English we can provide an interpreter.
- Our receptionists might ask when you arrived in the country but will not ask for immigration documents.
- If you are worried about giving us your address, let us know, your information is safe with us.
- If you do not have an address, let us know, we can help you register without fixed address.

An HC2 certificate entitles individuals on a low or no income to access free NHS prescriptions, dental treatment, wigs, sight tests, vouchers towards glasses or contact lenses and necessary travel costs to and from hospital for NHS treatment under the care of a consultant. For newly registering patients who may have had disrupted access to healthcare, we encourage you to have catch-up immunisations. This helps to protect you and our community. Please book an appointment with one of our nurses to talk through and provide what catch-up immunisations you need.

You do not need to know your NHS number to use NHS services, but it can be useful to have it.
Online Services
We recommend that you download the NHS App for your smartphone or tablet device. The NHS App allows you to seek medical advice, view aspects of your medical record as well as ordering repeat medication, viewing test results and booking appointments. If you do not have a smartphone or tablet device but still want access to online services, please email us or ask at reception access to online services. You will need to provide the reception team with your photo ID (passport or valid UK drivers licence) as proof.
Summary Care Record
A Summary Care Record is an electronic record containing key health information, which can be made available to NHS healthcare staff caring for you in an emergency or when your GP practice is closed.
Why do I need a Summary Care Record?
Storing information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
This information could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you.
Who can see it?
Only healthcare staff involved in your care can see your Summary Care Record.
Do I have to have one?
No, it is not compulsory. If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery. You can use the form at the foot of this page.
